Here you will find the paperwork needed for your initial session, along with other helpful documents for current clients.  You can print off intake paperwork and fill it out in advance, or you will be asked to fill out the paperwork at your first appointment.

 

What to expect on the Initial Session

Most insurance companies require an Assessment at your first session. These are also referred to as MSEs, PTAs or biopsychosocials.  This is a requirement we must follow mandated by several insurance companies.  Here at Therapeutic Choices, LLC we understand that you may not want to disclose certain information at your first appointment.  Therefore, we attempt to balance the insurance requirements, and meet your individual needs.  We may take more than one session to complete the required Assessment depending on your needs and emotional or cognitive state.
